Program & Administrative Coordinator (PACE Clinic)

Work Arrangement: Full Office

Schedule: Monday-Friday, 8:00 a.m.-4:30 p.m.

Assignment Duration: Up to six (6) months from start date

Role Overview

The Program & Administrative Coordinator supports daily clinic operations, specialty care coordination, and participant services within a mission-driven, high-performance environment. This role ensures smooth workflows, timely communication, and exceptional service delivery for participants and families.

Key Responsibilities

80% - Program Support

  • Contribute to a mission-aligned culture emphasizing customer service, consistency, dignity, and accountability.
  • Support and maintain Quality Initiatives related to specialty care services.
  • Provide reception coverage as needed to support clinic operations.
  • Coordinate transportation logistics with participants, caregivers, families, and contracted vendors.
  • Schedule specialty care appointments on behalf of participants.
  • Maintain proactive communication with participants and families regarding specialty services and follow-up needs.
15% - Administrative Support
  • Assist the team in executing departmental priorities and contribute to both short- and long-term goals.
  • Collaborate with administrative staff across the PACE center to ensure aligned workflows.
  • Manage confidential participant, personnel, and policy information with appropriate follow-up.
  • Coordinate specialty appointments and services in partnership with the clinical team.
  • Relay relevant updates to the clinical team and Interdisciplinary Team (IDT) to support timely completion of specialty care.
  • Document participant interactions in medical records according to established workflows.
Minimum Qualifications
  • High school diploma or equivalent and at least 1 year of experience in a clinical setting; or an equivalent combination of education and experience.
  • CPR and First Aid Certification required or must be obtained within six (6) months of hire.
Preferred Qualifications
  • Associate degree.
  • One (1) year of health care experience.
  • Bilingual in English and one of the organization's threshold languages: Arabic, Chinese, Farsi, Korean, Russian, Spanish, or Vietnamese.
