APN Healthcare Services Inc is seeking a local contract Mental Health for a local contract job in Newport Beach, California. Job Description & Requirements Specialty: Mental Health Discipline: Therapy Duration: 24 weeks 40 hours per week Shift: 8 hours, days Employment Type: Local Contract Position Title: Therapist MS (37175278) Location: Newport Beach, CA 92660 Duration: 6 months Hours per Week: 40 (8 hrs/day) Work Hours: ( M - F 8:30 am to 5 pm) Note: Will see patients in their homes but will report to office. Local Candidates only. Hospice Experience Required Duties: The Therapist MS provides comprehensive assessments, coordination, planning, and implementation of a psychosocial treatment plan for patients in respective department. (Community Health, Cares Palliative Care, Cancer Center Community Program etc.). This role aims to promote optimal mental health by providing ongoing psychosocial support and services to patients and their families for adjustments to illness, grief, addiction and other psychosocial stressors. Works with an interdisciplinary team to achieve treatment goals. Utilizes a variety of factors to evaluate patient's biopsychosocial needs. Develops a treatment plan and a comprehensive termination of treatment/discharge plan. Documents objective and accurate data regarding assessments and interventions. Maintains working knowledge of community-based resources, programs and social services. Educates patients and families on their health care options and connects them with appropriate resources. May develop and facilitate support groups, and workshops as needed. May serve as a resource and preceptor to MSW and MFT interns. May provide input on treatment schedules, patient assignments, and/or crisis interventions as requested by Clinical Manager. Performs other duties as assigned. Required Skills & Experience: Experience required varies by department. Department specific requirements are as follows: Community Health: -Experience in clinical assessments and crisis intervention as well as experience with culturally diverse patients. Hoag Orthopedic Institute (HOI): -Experience in an acute care setting. Preferred Skills & Experience: -Experience in clinical assessments and crisis intervention in a hospital or acute care setting. Community Health: -Bilingual or multilingual. Diabetes Center: -Minimum of two (2) years of MSW experience, preferably in an acute healthcare setting or equivalent work experience required. Required Education: -Bachelor's Degree. Education required varies by department. Department specific requirements are as follows: Community Health: -Master's degree in social work (MSW) or Marriage and Family Therapy (MFT) from an accredited school required. Hoag Orthopedic Institute (HOI): -Master's Degree in Social Work (MSW). Note: Will see patients in their homes but will report to office. Local Candidates only.
