Our team of professionals is committed to tailoring care plans that meet each patient's unique needs while offering guidance and compassion to their loved ones.Role DescriptionThis is a full-time, on-site role in North Coumty San Diego for a Hospice Liaison. The Hospice Liaison will work with healthcare providers to establish and maintain relationships, educate the community about hospice services, and help identify patients who may benefit from hospice care. Day-to-day tasks include representing the organization in the community, fostering relationships with referral sources, providing information about hospice and palliative care services, and collaborating with the care team to ensure the best possible patient outcomes.Compensation80-120 kBase with outstanding bonus structure, 401k , health insurance and other benefits.Qualifica tionsExpertise in Hospice Care, End-of-Life Care, and Palliative CareSales experience and ability to build strong partnerships with referral sourcesKnowledge and experience in Home Care and the broader healthcare systemOutstanding communication and interpersonal skillsAbility to work effectively in a team-centric, compassionate environmentProven organizational and time-management skillsExperience in community outreach and education is a plusCurrent licensure as a healthcare professional is preferred but not requiredminimum 2 year experience in Hospice Marketing/sale
