You'll follow an individualized care plan under the direction of a registered nurse or therapist, helping patients with daily tasks while preserving their dignity and independence.Key Responsibilities:Personal Care Assistance: Help patients with bathing, grooming, dressing, and toileting.Mobility Support: Assist with transfers, walking, exercises, and fall prevention.Monitor Vital Signs: Check and document temperature, pulse, and respiration during each visit.Meal Preparation: Assist with meals and feeding based on the patient's dietary plan.Medication Reminders: Prompt patients to take prescribed medications (no administration required).Light Housekeeping: Change bed linens, keep the environment tidy and safe.Observe & Report: Monitor patient condition and communicate changes to clinical staff.Follow Care Plan: Deliver care based on each patient's personalized home health care plan.Requirements:Current CHHA Certification through the California Department of Public Health.Reliable Transportation – You'll be traveling to patients' homes across Orange County.CPR Certification – Current and valid.Strong Communication Skills – Fluent in spoken and written English.Empathy & Professionalism – Compassionate, respectful, and dedicated to patient-centered care.Home Health Experience – Preferred, but not required.Ready to Make a Difference?If you're passionate about helping others live safely and independently at home, Remedy Home Health is the place for you.
