MemorialCare - - Responsibilities: Collect and document information pertinent to the patient's status in the electronic medical record and notify the registered nurse of changes in patient's condition; Demonstrates knowledge of the clinical and information technology systems; Follows directions and communicates with the registered nurse and other members of the healthcare team in an organized, clear and timely manner; Assists with activities of daily living and basic patient care; Maintains confidentiality of patient information
